Client Guidelines & Consent Form

This document outlines the terms, conditions, and guidelines for clients participating in iThrive's Root Cause Analysis (RCA), ALIVE program, and Advanced Test services. By proceeding, you declare that you have read, understood, and agreed to these terms.

1. General Consent & Authorization

Consent to Treatment: You acknowledge that the nature and purpose of iThrive's programs (including ALIVE!) have been explained to you, and you've had the opportunity to ask questions. You understand that a change in medical condition or medications can affect the treatment outlined by iThrive. You recognize the importance of notifying your coaches at iThrive of any changes in your health and agree to do so immediately.
Acute Conditions & Medical Emergencies: You understand that iThrive doesn't handle acute health conditions. You'll need a Primary Care Physician/Family Doctor to attend to any of your medical emergencies during your journey with iThrive.
Information Sharing: You agree and give consent to iThrive to access and share your details mentioned in the form, including personal details and medical records, with the team involved in your care and treatment. You also consent for this information to be used for research purposes.
Healing Modalities: You understand that iThrive ALIVE! is a different approach from the existing healthcare model. You understand it's the obligation of the iThrive team to identify difficulties you might be having with behaviors that interfere with your stated goals and to recommend additional healing modalities. You consent to receive recommendations for additional healing modalities.
Program Timelines & Lapses (ALIVE!): You understand and agree that the iThrive ALIVE! program will go according to fixed timelines and working hours. Any undue delay caused by you or your non-availability will lead to the lapse of the program. In such a situation, you understand and agree that there will be no further extension of the program, nor will you be eligible for a refund.
Release of Liability: You hereby release and discharge Thrivetribe Wellness Solutions Pvt Ltd from any potential claim, loss, or damage that you may suffer or have against iThrive as a result of, or occurring out of, or due to any treatment recommended by iThrive under their program in the event you suffer such Loss due to:
- Your non-adherence to the instructions and guidelines recommended by iThrive - Concealment (complete or partial) of any medical condition, history, treatment, or allergy(ies) that you may have or are suffering with.
Commitment to the Plan: You understand the key to healing is your commitment to comply with prescribed dietary changes, supplements, medications, and other treatment recommendations. If you don't follow the plan with reasonable consistency, your progress will likely be stalled. In such an event, you agree and confirm that iThrive may discontinue your treatment and ask you to unsubscribe from the program. In such an event, you agree that you won't be eligible for a refund as per the iThrive Refund Policy.  
Limitation of Liability: Thrivetribe Wellness Solutions Pvt Ltd won't be liable to you for any incidental, indirect, consequential, or special damages. iThrive’s total liability won't exceed, under any circumstances, the subscription amount paid by you to iThrive.
Dispute Resolution: You hereby agree that in the event of any dispute relating to your ALIVE! program, we'll first attempt to resolve the matter amicably through good-faith negotiations. If we're unable to reach a resolution through negotiation within 30 days, we'll appoint a common arbitrator and submit the matter to arbitration in accordance with the applicable laws and rules.
Public Comments: You hereby agree that you won't make any negative comments or reviews in public forums, social media, press releases, or any other medium about iThrive or their products, services, or business practices, whether by yourself or through your agents, during or after your program. You'll instead pursue amicable communications to help resolve any issues related to your program and follow the proper method of dispute resolution instead of such negative reviews and comments.
Jurisdiction: All further claims/disputes are subject to the exclusive jurisdiction of courts at Pune.

2. Partnership Guidelines (Applicable to ALIVE! & General Engagement)

Daily Tracker: Please update the tracker every day. It's the only way team iThrive can stay updated with your progress. You're expected to update the tracker every day for the 90 days that you're with us.
Core Working Hours: Our core working hours are from 9 am - 5 pm, Monday-Friday (Saturday 9 am - 1 pm). During this time, expect a real-time response to all your queries.
Emergency Contact: In case of an emergency during non-core hours, if you need an urgent response, please give us a missed call, and we'll revert to your query on chat.
Sundays: Sundays are off days, and we usually don't check your chat messages. In case of an emergency on a Sunday, if you need an urgent response, please give us a missed call, and we'll revert to your query on chat.
International Clients: For international clients, we'll, to the best of our ability, offer real-time responses depending on your time zone.
Life Situations: Please keep us informed if there are unanticipated life situations like travel, hospitalization of a family member, ceremonies, and family events, etc. Team iThrive is here to coach and guide you through these challenges so that you can stay on track despite them, as straying off ALIVE! is in all likelihood detrimental to your health.
Program Integrity: ALIVE! cannot be modified to accommodate cheat days, alcohol consumption, or any other unhealthy practices that harm your health. Please avoid requesting these.
Communication Tone: Your 3 months with iThrive is a journey filled with learning, growth, and fun. We connect with you from the heart and soul, and a tone of respect and peaceful communication is what we'd expect from you. We truly encourage genuine curiosity, and we hold the space for us to together learn more and more about your health. An attitude of learning is welcome and appreciated.
Respectful Conduct: We expect you to be courteous and respectful towards everyone on the team; this includes being kind, compassionate, and respectful about your own self.
Raising Standards: We'll continuously raise standards for your health. Please flow with us and don't resist these high standards by indulging in self-abusive patterns.
Disrespect/Mistrust: Do note if we perceive disrespect and mistrust towards your coaches, we'll point it out to you. If the behavior isn't amended, we may discontinue your treatment and ask you to unsubscribe from the program. In such an event, you agree that you won't be eligible for a refund as per iThrive's refund policy.
iThrive Tribe Group: You're welcome to share recipes, tips, and ask general queries and learn from the discussions posted on the iThrive Tribe. Queries specific to your own protocol are only to be posted on your personal group. Please do maintain a tone of respect and courtesy towards fellow members of the tribe so that we can learn and grow together.
